As a licensed clinician, Jonathan Baiden, LCSW-C provides a range of clinical letters, assessments, and documentation for clients who need formal written support. All documents are completed at a fee and require an existing or established clinical relationship.

Emotional support animal (ESA) letter

A formal ESA letter completed by a licensed clinician for clients with a documented mental health need. Accepted by most housing providers and airlines.

Clinical support for Family and Medical Leave Act paperwork requiring a mental health provider's assessment and signature.

Letters supporting workplace or academic accommodations for clients with documented mental health conditions, prepared for HR, ADA, or school use.
